This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

The law requires us to keep your medical information private, to give you this notice of our duties and privacy practices, and to follow the terms of the notice currently in effect. How we may use and share your information

There are everyday situations where we use your health information to care for you and run the practice. The most common ones are:

For your treatment

We use your information to provide and coordinate your care. For example, your physician may share relevant details with a specialist we refer you to, a lab that runs your tests, or a pharmacy that fills your prescription.

For payment

We may use and share your information to bill and collect payment from you, an insurer, or Medicare, and to confirm coverage before a visit or procedure.

For health care operations

We may use your information to run the office well, such as reviewing the quality of care, training staff, and scheduling. We may also remind you about appointments or tell you about treatment options and services that may interest you.

Other uses allowed or required by law

In certain situations, the law lets us, or requires us, to share information without your written permission. These include public health and safety activities, reporting suspected abuse or neglect, responding to a court order or lawful request, helping with workers' compensation claims, and cooperating with health oversight agencies. We share only what is needed for the situation.

Uses that always need your permission

We will ask for your written authorization before we use your information for marketing that benefits a third party, before we sell your information (which we do not do), and before sharing most psychotherapy notes. You may take back that permission in writing at any time, and we will stop, except where we have already acted on it.

Your rights

• See and get a copy of your medical and billing records.
• Ask us to correct information you believe is wrong or incomplete.
• Ask for a list of certain disclosures we have made of your information.
• Request a restriction on how we use or share your information. If you pay out of pocket in full for a service, you can ask us not to share that information with your health plan.
• Ask us to reach you a certain way or at a certain place, such as by a specific phone number.
• Get a paper copy of this notice, even if you agreed to receive it electronically.
• Be told if a breach affects the privacy of your information.

Our responsibilities

We are required by law to protect your information, to notify you if a breach occurs, and to follow this notice. We will not use or share your information other than as described here unless you tell us we can in writing.

We may update this notice and apply the changes to all information we hold. The current version is always available at our office and on this website, with its effective date shown at the top.

If you feel your privacy rights have been violated, you may file a complaint with our office at no cost and without fear of retaliation. You may also contact the U.S. Department of Health and Human Services, Office for Civil Rights.
